Helpful Tips for Data Submission

This guide is designed to assist you in completing case report forms, edit/clarification reports, supplemental corrections, and various other study related tasks. We are committed to helping you collect the best data possible in a timely manner.

Print legibly - Remember, data processing staff are not medical personnel! If we cannot read your handwriting, data accuracy is compromised.

Staple all multi-page forms - if they are not stapled together, the possibility of pages being out of order and/or missing, or mixed with a different patient's form increases.

Collate all forms in ascending patient order. Be sure to list them on the Data Mailing Checklist. Include Form Number, Patient Number and Visit.

When submitting Supplemental Corrections be sure to include the Visit Number.

Remember to record Patient Number, Reference Code, and Site Number on the first page of Informed Consent forms.

Refer to your copy of the Variable Reference Guide (VRG) when completing Edit/ Clarification Reports and Supplemental Corrections. When making changes be sure to add the change to your copy of the form.

Don't forget--initial and date changes.

Remember to review "patient-completed" forms for completeness before submitting to Perry Point.

All numbers should be entered "right-justified" with leading zeros, so that each space for a response has a number in it. This includes responses that have decimal places.

"Circle all that apply.."

Some forms have questions that ask you to "circle all that apply." Keep in mind that each answer is its own variable. When you select an item, we key it as a "1". When completing edit/clarification reports and supplemental corrections be sure to reference your VRG for the correct variable names.
